    Today, the PUL community is reeling. Stephen MacDougall was a mighty opponent on the ultimate field, but to a great many, he was an even mightier friend. Stephen's character and passion, as a player and as a coach with Juniors, showed exactly the qualities that continue to make PUL great. His loss is almost impossible to comprehend, and our hearts are with Kim, Rielle, and Kieran and all those who were close with Stephen. PUL will certainly do something formal to acknowledge Stephen's contributions, but in the meantime, folks should feel welcome to post any memories or photos below. RIP, Stephen. You will be dearly missed.

    Stephen MacDougall has fought so hard to stay with us, but his body is getting tired. Rielle, Kieran and I are here at the hospital with him and are now just outside his window in the shade of the ambulance bay getting some air with some friends showing their love. He is so well loved and at peace. We are heartbroken. 💔 Anyone is welcome to stop by or drive by.

…and I spent the night here. Stephen MacDougall had been having some oxygen desaturation episodes yesterday and his BP was getting progressively worse, despite increased pressor drugs throughout the afternoon and evening. Stephen had ultrasounds, xrays and a contrast CT to try to see what was going on and look for another pneumothorax (air pocket leaked outside lung) or a blood clot. Nothing showed up on imaging. Dr. Ball, the cardiologist on call, had come to see if the pericardial effusion (fluid around the heart, present from the other day) had become larger or was pressing on the heart, but that did not seem to be the case. Stephen's heart was strong, 'snappy' and appeared normal. Yay! It was unclear what could be causing this. Coming into the evening was increasingly scary. I am most afraid of the nights. By 3am Dr. Ball was back to insert a drain around Stephen's heart. He didn't think think this would improve his BP, but the team was at a loss and wanted to rule it out. Dr. Ball was right, unfortunately. He did, however, get all the fluid out and removed the drain successfully. Pressor drugs continued to be upped to the point where Stephen was on 20 levo, plus vasopressin and 15 epinephrine. A bolus of fluid was put through his IV in case the BP issue was related to low volume. Stephen's heart rate was increasing (likely from the epi) and he now had a low grade fever. Dr. Rice suspected a possible new infection and started Stephen on antibiotics. As the morning went on, the BP began to improve and the epi was weaned to where it now stands at 5, with the other pressors still in use. Stephen is oxygenating well with good saturation at 50% and slightly decreased pressure and peep as compared to yesterday. Fever went back down. Who knows what caused all this. I'm just trying to hang on for the ride. I'm hopeful this BP improvement will continue and Stephen can again be weaned off of the BP support drugs soon, so we can return to where we were yesterday morning when the team thought he was looking good and on the mend. When all this is over, I'm going to make some therapist a lot of $$. Thanks for all the love and support. ❤️

I'm still here with Stephen MacDougall and it's been a quiet day, aside from the playlist and your video stories which I'm playing for him, but quiet is good. Yesterday after much research, very careful and methodical examination of Stephen's various markers and their trajectories and consultation with others, Dr. Greg posited that a stronger course of hydrocortisone steroids might likely help reduce Stephen's multi-system inflammation. Everything has to be weighed in balance, as every change with positive outcomes comes with risks and often negative consequences as well. Pulling fluid is necessary to reduce swelling and inflammation, but works in opposition to blood pressure gains, for example. Steroids in high doses present risks of infection in the longer term, and could fuel new infections if they are present. Every single thing needs to be monitored so closely. So Stephen started the steroids again yesterday, in much higher doses than before, and the team is cautiously optimistic that they are helping. They have been able to titrate down the blood pressure presser drugs so that he is now off levo and vasopressin, and epinephrine is down to 5 mcg/min. Yesterday and in previous days he was needing A LOT of BP support. Stephen's oxygen and ventilator settings remain unchanged with 94-95 saturation on 40% O2 (pt 12, peep 18 still). His heart rate has been regular around 100. Sedation has been titrated down to 1 mg/hr midazolam (versed) and fentanyl 25 mcg/hr. Dr. Greg noted some neurological improvement today as well, with Stephen's pupils more reactive and less asymmetrical. Stephen's lungs are the same, with no pneumothorax (pocket of air leaked outside the lungs, as previous). Continuous dialysis is still running and pulling fluid at around 100cc/hr to help with swelling. There is still some fluid around his heart (pericardial effusion), which they are keeping an eye on but not enough to insert a drain again. No fevers. Somewhat concerning is that blood tests looking at Stephen's liver are showing that it is now more inflamed, which Dr. Greg thinks is likely a shock reaction resulting from the (necessary) increase in BP presser drugs 3-4 days ago (which impede blood flow to and from liver), and is hopeful that their reduction will help this improve. An ultrasound of the liver was done to rule out any obstructions (not present). Stephen is now getting small amounts of insulin as high dose steroids affect his blood sugars, but this is temporary. Stephen's vitals are improving to the point that he could tolerate switching his ventilator to a tracheostomy insertion, which sounds scary but actually makes extubating (removal of ventilator) much easier and safer. The trach can be capped so spontaneous breathing trials are easier, when he wakes he can talk, and they can better adjust to his oxygenation needs. This was something they were looking to do about a week ago before he again got sicker, making it too risky. This will likely happen when Dr. Mark returns on Monday, as he performs these procedures here in the ICU. So overall a good day. Stephen's certainly not out of the woods yet, but right now his team is 'generally happy' with his progress. As I finish writing this, the respiratory therapist has just adjusted the ventilator settings down to 35% oxygen and pressure (peep) reduced to 17, while also moving Stephen onto the spontaneous breathing setting where he initiates his own (supported) breaths, and he's tolerating it well with saturation of 92-93. Thanks, everyone, for all your love and support, and for sticking with me on this long journey. Let's hope for another uneventful night. ❤️ P. S. - As an aside, for those here in Peterborough, I meant to share this last week if any of you happen to drive by and wonder where to direct your positive vibes. 😉 A3 ICU is on the same level as Emergency, located just behind the ambulance bay and overlooking the not-currently-in-use North Entrance (oddly, one floor above it). Window faces west, nearest to the inside corner on the grey exterior part of the building. Clear as mud! If you get close enough (walking up to the entrance doors) you can see Stephen's pink Fu€k Cancer cycling Jersey, which arrived in the mail from Cheryl Dove near the start of all this, hanging in the top right corner of the window, in preparation for their 100 Mile (160km) Ride to Conquer Cancer. It's much easier to spot at night. 💕

…I sit here nervously at Stephen MacDougall's bedside, willing things to improve with all of my being. He is still in a coma, less sedated now (1 mg versed, 37.5 mcg fentanyl), from a terrible but still unknown virus that hit suddenly and aggressively, causing multi-system failure. It's hard to fathom how a healthy, active 45 year old could be out biking 50 km one day, then go to work the next morning only to return home at lunch time in debilitating pain and with a dangerously high fever, which soon progressed with heart failure, lung failure and kidney failure among other issues. Stephen is on a ventilator and continuous dialysis in the ICU, going on 19 days now. It seems he is over the initial virus and its accompanying 41-42 degree fevers, but his organs have been injured and need time to heal. In this process, since the initial virus passed, he's had to overcome a whole body blistering rash, unbelievable swelling and dangerous fluid retention, a pneumothorax (air pocket leaking into the membranes outside the lungs), a pericardial effusion (buildup of fluid surrounding and compressing heart), oxygen desaturation episodes, manual resuscitation, liver and pancreatic involvement and more. He just seems to have all the things. Despite one of the most expensive workups possible, and multiple consults with other centres to ensure nothing is being missed or could be done differently, Stephen continues to baffle this amazing team. He is receiving excellent care, and despite not knowing what has caused all of this, the treatment is likely the same: what's called supportive care, which is what he is receiving. Today Stephen's vitals are relatively unchanged from yesterday. He's on 35% oxygen on the vent (pt 12, peep 18), saturation 95. Blood pressure support is up a bit with levo at 22 and vasopressin on. Dr. Greg just did a bedside ultrasound to look at his heart, and although there is a bit of fluid surrounding it, it does not appear to be compressing on the ventricle. A formal ECG later will check for sure. Thank you all for your continued prayers and positive energy. It's been a long road but I am hopeful that Stephen will be turning the corner soon. ❤️

He was out of practice, said the reeds were dried up and drones out of tune, but I think it sounds pretty damned good. And it gives me some comfort right now, and maybe you, too. November 10, 2020.
